As a reminder, it’s best practice to first search by DOB before manually adding a new patient chart. Even when
being careful, there are still various scenarios can lead to duplicate charts in the system. One example could be from where a chart was created via an interface then a duplicate
chart was created manually. Another example could be where a patient had duplicate charts with a different last name or first name. In any case, the system makes it easy to find
and merge duplicate charts. Or, in the instance where patients with similar names have been merged, the system has a mechanism to un-merge those records that were combined in
error.
